Central Illinois Resource Center Inc (“CIRC Inc”) is a 501(c)(3) nonprofit organization. Our Peer-to-Peer Support Line provides non-clinical, peer-based emotional support and resource guidance. Volunteers are not acting as licensed mental health professionals, medical providers, attorneys, or financial advisors, and the support line does not provide diagnosis, treatment, or medical or legal services. The information submitted through this application is collected solely for volunteer screening, communication, and internal record-keeping. Your information may be shared internally with CIRC Inc board members, officers, and designated committee members on a need-to-know basis. CIRC Inc does not sell, rent, or disclose applicant information to third parties. Submitting this form does not create an employment relationship, contractual appointment, or guarantee placement as a volunteer.
